South East Toronto Family Health Team

Affiliated with the University of Toronto Department of Family and Community Medicine and funded through the Ministry of Health and Long-Term Care, the team of highly specialized health care professionals at SETFHT provide primary care services to roughly 20,000 patients in the East York area. With a strong community focus and multi-disciplinary approach to the provision of primary health care services, SETFHT seeks to improve access to care while producing better patient outcomes for the whole family. The team consists of the following : Family Physician; Medical Residents; Nurse Practitioner; Registered Nurse; Pharmacist; Registered Dietitian; Mental Health Addictions Counsellor; Social Worker; Clinical Psychologist; Care Navigator; Chiropodist; Physician Assistant; Health Care Professional Students / Interns; and Administrative and Clinical Support Team Members.
